What to Expect from Your First Appointment: 

Your first visit is an opportunity to review your past history and to learn what health concerns you have. A detailed examination is done and recommendations to optimize your health may be discussed. We have a laboratory drawing station in the building and excellent radiology facilities nearby.

Office Policies and Procedures 

Please arrive 15 minutes before your appointment time to complete an initial history form. Please review your family history and your shot record before your visit. If you have any records of significant health issues, please bring them. Your insurance card is needed if you would like us to bill your insurance. If your insurance requires that you use specific laboratory or radiology facilities, be sure to notify us.

If you need to cancel your appointment, please give 24 hour notice so that we may provide that time to another patient.
